Direct Care Professionals

Our nursing staff will complete a thorough assessment upon arrival, taking into account medical records that have been received from the resident’s primary caregiver (doctor, hospital and/or other facility). All orders will be verified with our staff doctors, whom are available to continue care while your loved one is at our facility. The professionals will follow the care plan prescribed based upon the assessment performed, as we are dedicated to each resident’s health and well-being.

Social Services

Our social services team will work with you and your loved one to determine the most cost-effective options available. We discuss factors such as Medicare, Medicaid, long-term care insurance and veteran’s benefits. Our goal is to enable a smooth transition into our community so we work with all professionals at the facility to address your loved one’s specific needs and are available to answer questions regarding the resident’s stay. Additionally, we will provide assistance with all of the resident’s care needs upon discharge from our facility.

Three Levels of Care

Assisted Living

Assisted Living is for those who may require assistance with medication administration, bathing, personal grooming, meal preparation, laundry and housekeeping. Each resident’s care plan is personalized to ensure activities, independence and assistance are all balanced for a healthy lifestyle. Assisted Living provides your loved one with the opportunity to interact with peers through day-to-day relations, planned activities and events. Residents can focus on socializing, have fun and building relationships while still feeling at ease knowing support is available if needed. While still valuing the independence of our residents, we always want to assist them in living their most comfortable life.

Skilled and Intermediate Nursing Care

All residents are provided daily nursing services that are directed by physician’s orders and treatment plans.

Skilled nursing provides short-term care to residents focused on recovery and rehabilitation so they can return to their normal lives as quickly as possible. These services are often utilized during recuperation from serious health issues and are not able to be cared for at home.

Intermediate care is utilized for residents that do not require daily medical treatment but are unable to sufficiently care for themselves at home. This options provides light nursing care and assistance with daily activities. This is often the type of care that is pursued when looking for a balance of independence with an appropriate level of care, found in nursing homes.

Respite Care

This service provides an alternative to caregivers that need a break from the daily responsibilities of caring for their loved ones. This can be a great opportunity for your loved one if they require supportive services but still enjoy engaging in social interactions and activities.

Therapy

Based on your loved one’s physician’s orders, our therapists will perform an evaluation to identify if physical, occupational or speech therapy would be beneficial. The therapy team prepares an individualized plan of care for each resident to achieve their maximum functional level and to move towards recovery
